Practicum Leader’s Evaluation Form

In order for the student to receive credit for Practicum Ministry, they are to have attended at least 75% of their practicum hours (30 out of 40 hours) and receive a “satisfactory” mark from their Practicum Leader. Rate the student in each of the following areas from 1 (very poor) to 10  (excellent), and then enter the total by adding the points from each area. A total of 70 points or higher is required for the student to receive a “satisfactory” mark.
